Errata (fixed in second printing; see note on errata page which demonstrates a jump to this page.):

On page 762, below the second grey box, the form of the JUMP-TO-NAME anchor should be:

< A NAME="JUMP-TO-NAME" > Something < /A >

Where Something = a text element in the file such as a word or phrase. Without that "Something," the anchor does not work.
